In-office Review of Medical Records
You may review your confidential health information in the HIM department during business hours by appointment only.
Obtaining a Copy of a Medical Record
In most situations, University of Colorado Hospital must get your written permission to release a copy of your confidential health information. We cannot accept e-mail requests.
Please print and complete the request form below. Your handwritten signature is required for us to complete your request.

Mail or fax your completed, signed request form directly to the HIM department. The address and fax number are listed on the form and at the top of this page. You also may drop off forms in the HIM department during business hours.
Note: We can only provide you a copy of the information in your record. We cannot interpret what the information means or discuss it with you. Please take the information to your personal doctor to discuss the findings.

Receiving Your Requested Records
Once the HIM department receives your authorization to release information, it takes 5 to 10 business days for the record to be photocopied and mailed to the address you provide. Please bring a photo ID if you choose to pick up your medical record copies.
To protect patient confidentiality, we cannot fax confidential health information directly to patients or businesses. We can fax this information directly to a doctor’s office or other health care facility.
Payment for Records
University of Colorado Hospital is authorized by Colorado State Law to charge the following fees for copying medical records:
$14 for copying the first 10 pages
$.50 per page for the next 11-40 pages
$.33 for each additional page after the first 40 pages
You will receive an invoice with your protected health information. Payment is due upon receipt of this invoice.
As a courtesy to its patients, University of Colorado Hospital does not charge for the first 10 pages copied.
Advice when Requesting Copies of Medical Records
Requests for “all” information (including progress notes, consent forms, registration sheets, etc.) can delay processing, become very costly and is often much more information than you really need.
If you need help deciding what types of reports to request, please contact the HIM department.
Requesting an Amendment to a Medical Record
Review your records carefully. If you feel something needs to be revised or corrected, please complete the Request Amendment to Medical Record form below and mail it to the Privacy Officer in the Health Information Management department at University of Colorado Hospital. Please contact the HIM department if you need additional information about this process.
