Congratulations on your job offer from the University of Colorado Hospital!


Now, as a prospective new employee, you will need to complete a post-offer, pre-employment health screen evaluation prior to your hire date. PLEASE NOTE: ONLY INDIVIDUALS WHO HAVE BEEN OFFERED A POSITION SHOULD SUBMIT A HEALTH HISTORY FORM.

Please follow the steps below to begin this process

  • Complete the health screen form (found via the link below) and return to UCH Employee Health within 24 hours of receipt
  • Email to eHealth@UCH.edu or fax to 720-848-7376
  • The Employee Health Nurse will contact you by phone 24-48 hours after receipt of the completed form.  A nurse will call you to review your health history. Please make sure that we have your updated phone number (cell phone & home phone preferred)
  • Your appointment in the Employee Health Clinic will be scheduled prior to your orientation and hire date
  • If you have any questions regarding the health screen contact Employee Health at 720-848-6849.

Special Notes:

  • If you are moving to Denver you must be available for the clinic visit three business days prior to your scheduled orientation. You will not be able to attend orientation until you have completed the health screen process.

Please keep in mind that you are not hired until you have been cleared through the health screening process. Employee Health will communicate with you when you have cleared your health screen and will set up an in person visit to their clinic prior to orientation. Please check your e-mail for orientation information.
