Changing Your Benefits
Changes can be made during open enrollment, or when you have experienced a "Qualifying Life Event."
A Qualifying Life Event is an event such as marriage, commencement of domestic partnership, divorce, termination of domestic partnership, birth or adoption of a child, a child becoming an ineligible dependent, your spouse or domestic partner’s loss or gain of coverage because of a change of employment, going on or returning from leave, or a change to part-time employment or vice versa.

If you have experienced one of these changes or wish to add or remove a dependent as a result, you must fill out the Qualifying Life Event form and fax it to the the Benefits Department within 31 days of when the Qualifying Life Event occurred. Fax the form to 720-848-6802.
Adding a newborn?
Just contact the HR Service Center at 720-848-6800. If you are enrolled in PacifiCare health benefits, please have your new child's PCP's information handy.
