- 500 word personal statement addressing why you are interested in the Fellowship and what your goals are for the 12 month position
- Three letters of recommendation (at least one academic and one professional/direct supervisor)

UCH is a 410 bed hospital, which admits over 20,000 patients and sees over 700,000 outpatient visits per year.
Our $400 million 12 floor expansion tower, to be completed in Spring 2013, will increase inpatient capacity with space for another 280 beds, additional ORs, interventional suites and a conference center. A new Emergency Department will be equipped to accommodate 85,000 visits per year.
