What to Bring to Your Executive Health Program Appointment

  • A list of current medications
  • Previous medical records. You also can arrange to have them transferred prior to your scheduled appointment

Executive Health Program Insurance and Payment

The Executive Health Program is a fee-for-service program. We are not affiliated with, bill or collect from any insurance carriers, managed care plans or Medicare.

Upon your request, we can provide bills that you may use to file claims with your health plan. Any applicable reimbursements should be paid directly to you. We do not accept reimbursement or payment from any private or commercial insurance plans. All patients are legally responsible for all charges in connection with the medical care and treatment provided by the representatives of the program, including any unforeseen hospital stays or additional services not included in the EHP package.
