Adult Cancer Survivor Clinic at
University of Colorado Hospital

The Adult Cancer Survivor Clinic, a collaboration between the University of Colorado Cancer Center (UCCC) and the hospital's Department of Internal Medicine, helps adult survivors of cancer follow a path to proper medical care in their post-cancer lives.

Survivorship care plans for patients

Adult cancer survivors share certain health issues, including diabetes and heart problems. In addition, many suffer distress caused by the fear of their cancer recurring.

To address these concerns, the clinic provides patients "suvivorship care plans" that serve as roadmaps for future care. These care plans follow guidelines set forth in "From Cancer Patient to Cancer Survivor: Lost in Transition," a landmark 2005 report by the National Academies' Institute of Medicine.

The report says that care plans should contain:

  • Diagnosis and treatment information (including potential treatment consequences);
  • The timing and content of follow-up visits;
  • Tips on maintaining a healthy lifestyle and preventing recurrent or new cancers;
  • Legal rights affecting employment and insurance; and
  • The availability of psychological and support services.

About the clinic

The Adult Cancer Survivor Clinic is part of a major Lance Armstrong Foundation effort to improve cancer survivorship programs across the country. UCH is launching the clinic with help from a five-year, $1.7 million Armstrong Foundation grant.

Providing expert patient care at the clinic are:

  • Linda Overholser, MD, internist;
  • Nivedita Mahidhara, MD, internist;
  • Jana Bolduan Lomax, PsyD, clinical psychologist;
  • Kristin Kilbourn, PhD, clinical psychologist; and
  • Alison Jones, ND, RN, nurse educator.

The clinic meets on the third Thursday of each month in the hospital's Department of Internal Medicine, located on the fifth floor of the Anschutz Outpatient Pavilion (see maps and locations ).
