What to bring to your appointment
Please bring the following information with you to your appointment. We can assist you in obtaining these records if necessary:
- Past medical records related to your diagnosis (such as reports of the operation and any chemotherapy records)
- X-rays films and reports, such as CT and PET scans related to your diagnosis. These are frequently placed on computer CDs
- Pathology slides
- Radiation therapy records, if applicable
Plan to arrive 15 to 20 minutes before your appointment to complete clinic and insurance forms.
